All because of a paperwork issue
A Blackpool man alone died by choking on a sandwich in hospital after his care home failed to pass on that he could only eat blended food.
Mr Burns was a resident of Highgrove Rest Home in South Shore at the time and was only given blended food due to a history of food aspiration. This is where an individual accidentally inhales food through vocal cords into the airway. On the day he died, Mr Burns' physical condition had deteriorated and paramedics from North West Ambulance Service had been called to take him to A&E at The Vic.
