Molly Partington, from Bury, was just 18 when she died on August 17 last year after leaving Stubble Bank, a specialist residential mental health service, for a walk and falling from a bridge
A ‘bubbly and caring’ teenager described by her family as the ‘life and soul’ should not have been discharged from hospital the night before she died without a mental health assessment, a coroner has found.Following a two-week inquest into Molly's death at Rochdale Coroners' Court, senior coroner Joanne Kearsley concluded the 18-year-old's cause of death was multiple fatal injuries from an intentional fall from height with complex trauma and multiple emotional personality disorder.
When she returned to Stubble Bank Molly made repeated comments that she wanted to die and said she wanted to go to the bridge. An incident form was completed by a support worker and the comments were noted. Support staff on duty the following day were not told about Molly’s comments and no one at Stubble Bank read or assessed the incident report, the inquest heard.
Following the review Molly went back to her room and asked for a support worker. The support worker found Molly on her bedroom floor. She had reopened the stitches of her leg wound, the inquest heard. Ms Kearsley said it was possible that had the support worker known about Molly’s comment she may have tried to stop Molly by locking the door.
Ms Kearsley said she was satisfied that by 4pm there were missed opportunities by Care in Mind, which runs Stubble Bank.
