Robert Allen died at Wishaw General Hospital in 2019 after taking a quantity of Methadone and Quetiapine that he was believed to have been stockpiling in his cell at Shotts prison.
Robert Allen was housed in the jail's segregation and reintegration unit in the months leading up to his death after two incidents of serious violence, a Fatal Accident Inquiry was told.
He had been prescribed anti-psychotic medication following a deterioration in his mental state which he was taking alongside his daily dose of Methadone. A decision was also made to dispense Mr Allen's medication, including his high dosage antipsychotic medication, on a weekly basis unsupervised. Despite Mr Allen's previous history of repeated episodes of intoxication with unknown substances - including several incidents where he required a medical emergency response - there were no searches conducted with an NHS worker present. It was heard that an NHS colleague could have identified any potential hoarding of medications.
Although, another doctor suggested that given that Mr Allen was known to not be taking medications, there would have been reasonable grounds for a weekly search.
