Wayne Hemmings, 47, had been in an 'agitated state', a coroner has ruled
A man with a history of mental health problems died after he fell from the edge of a quarry, an inquest heard. Wayne Hemmings, 47, suffered catastrophic injuries when he fell into a garden.
He was assessed by mental health professionals in May 2021 when it was concluded he was of 'low' risk of harming himself, the inquest was told. The inquest heard Mr Hemmings was concerned that he had not received his medication but a toxicology report revealed aripiprazole was in his system at the time of his death.
Coroner Catherine McKenna noted evidence from his family that Mr Hemmings visited the spot as a child and played on a rope swing there.
